2. Turn in the following items to register your child:
⦁ Completed and signed Registration Form
⦁ $125 non-refundable Registration fee
⦁ Current copy of Immunization record, as required by the Indiana Dept. of Health
⦁ Copy of any relevant court orders (if applicable)
